Scheduling an Appointment

Office visits are by appointment only.

(We do not accept appointments online or by email at this time.)

Appointment Time:
Please arrive 15 minutes prior to your scheduled appointment to allow time for registration. To save time, you may download the applicable registration forms at home, fill them out and bring them to your appointment.

Late Policy:
If you arrive more than twenty (20) minutes late to your scheduled appointment, the front desk agents will assist you in rescheduling your appointment for a future time. This is the only way we can ensure that there is adequate time for your physician to complete your consultation. Rescheduling your appointment will allow your physician the opportunity to fully optimize your time to successfully benefit you during your appointment.

Cancellation Policy:
In order to aviod a cancellation fee, appointment cancellations must be made at least 24 hours prior to your scheduled appointment. They may be made by calling 727-869-7822 during our business hours:
Monday through Friday 8:30 am - 4 pm.
